(As stated above all these questions except for 'pairing' and 'fandom' are optional. Please copy and paste to your comment and just delete the non bolded parts and type in your answers. The non-bolded is more of a guide for you).

Nickname: What do you want to be called. This is for communication and also for when I tell who the fic is dedicated to...

Age: Just let me know what reading level you're at. I don't want any 11 year olds going NC-17 sounds cool!
 -_- I don't mind people reading whatever they want, it's your life so whatever however I tend to not write anything too graphic for super young ones. If you want just put that you can read NC-17 so I don't wonder.

Fandom: The list here tells of all the shows I'm familiar enough with to write about- or at least try to write. However I can write an original fic if you want- if that is it then please type original (and don't forget to answer the question in italics). I just wanna know what show first...

Pairing: Any is fine no matter what type (I'm well known for crack pairings), but I can't write the pariring request if ya don't tell me! If you want an OC pairing that is cool just go to where the italic question is or I'll be bugging you!

Rating: Anything from G-NC-17 is allowed. If you don't know for sure what rating level just say something to the effect of- nothing over (insert rating), so I don't write something you hate.

Genre: What kind of genres do you like? What ones do you hate? Or do you have a certain idea for the story? Tell me here and I'll do it- or avoid it depending on what you want. I can do any genre- though true romancy I do not do as well, however I will do it for you!

Plot: This is only if you know of a plot. If you're going for the whole just wanting to request a pairing leave this blank and I'll think of something (but the rating level doesn't go over PG-13 unless you tell me you want it to.)

Warnings: Haha ok this only applies to people that want content in the fic that may be sexual in nature or have violence or things like that. Pretty much if you want anything R or above then give a list. This also is another way of saying- what do you like to read.

Story length: Do you have a preference? One-shot or multi-chaptered are fine. I just want to make sure that you don't want a one-shot and I make it a long story or vice versa. If you really don't care just leave this blank.

Extra: Anything extra you wanna add? Do you have things you hate in fics? Any types of genres you hate? Hate angst? Hate happy endings? This is the place to tell me, otherwise I'm gonna assume you don't hate things in fics...

Characters: This is for just original (or OC pairing) fics, this is where you tell me about any characters that you want in your fic- otherwise they'll be ones I think of. If you have characters you want tell me all about them- even if it's just one character. If you want me to come up with the characters for the fic just leave this blank. OC characters however must fill this out (include name, age, personality, looks, normal attitude, any special powers/features, attitudes towards other characters in series [for example Yu Yu Hakusho fic with an Kuwabara/OC pairing- the OC is combative to Hiei, annoyed of Kurama and treats Yusuke like an annoying older brother, wants to slap Keiko for yelling so much- or whatever like that])!!!
